Services

Iowa IV LLC provides IV hydration therapy, injection services, peptide programs, and related wellness services administered by registered nurses and supervised by licensed medical professionals. All services are subject to clinical review and may be declined if deemed medically inappropriate.

Medical Disclaimer

The services provided by Iowa IV have not been evaluated by the FDA and are not intended to diagnose, treat, cure, or prevent any disease or medical condition. IV therapy and related services are wellness services and are not a substitute for professional medical care. Always consult your physician before beginning any therapy program.

Eligibility

You must be 18 years of age or older to receive services. Certain health conditions may disqualify you from receiving specific treatments. Our clinical team reserves the right to decline service to anyone for medical safety reasons.

Appointments and Cancellations

We request at least 24 hours notice for appointment cancellations. Late cancellations or no-shows may be subject to a cancellation fee. Walk-in appointments are accepted based on availability.

Memberships

Iowa IV membership plans are billed monthly and may be cancelled at any time with no contracts. Membership benefits are non-transferable and valid only at Iowa IV locations. Pricing and benefits may be modified with reasonable advance notice.

Payment

Payment is due at time of service. We accept all major credit cards. Iowa IV is not responsible for any health insurance claims or reimbursements.

Limitation of Liability

To the fullest extent permitted by law, Iowa IV LLC shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of our services or website. Our total liability shall not exceed the amount paid for the specific service in question.

Governing Law

These Terms of Service are governed by the laws of the State of Iowa. Any disputes shall be resolved in the courts of Polk County, Iowa.

Changes to Terms

Continued use of our services after any changes to these Terms constitutes acceptance of the revised terms.
